“Surprise Bill” Law


Dear Patient,

April 1, 2015, New York State enacted what has become known as the “Surprise Bill” Law. This bill requires The New York Eye Surgical Center to provide patients with certain disclosures prior to the provision of non-emergency surgery. You have the right to know what will be billed for your surgical procedure and, if you request, the anticipated cost.


  • American Progressive Life & Health – Medicare Advantage, PPO & HMO Benefit Plans
  • BC BS of Northeastern NY (all products including Commercial, Managed Medicare and Managed Medicaid)
  • CDPHP (all products)
  • Empire BC BS (all products)
  • MVP (Select Care, Medicare Advantage, Medicaid)
  • Today’s Options of NY – Medicare Advantage, HMO & SNP
  • Universal American – Medicare Advantage, Special Needs Program and PFFS Benefit Plans
  • United Healthcare (all products)
  • Fidelis
  • Humana

Even though we may participate with your insurance company, you may still be responsible for a portion of the bill depending on the terms of your policy. Participating with an insurance simply means that, in most cases, the facility and insurance company have an agreement for payment.


All other insurances not listed are billed as a courtesy to our patients. However, they are billed out as a non-participating provider. A surgery center will call you with the $$ amount due at the time of service and is available, upon request, by calling 518-450-1304.


PLEASE NOTE: Services for anesthesia WILL BE BILLED SEPARATELY by JJM Medical Services, PC (see below)
